If you want to collect information from people who book appointments with you, you can do so on the Booking Link Setup. You can also specify what information you want from your customers. When customers book appointments via your Booking Link, they will be asked to provide whatever information you’ve required when setting up the Booking Link, which will be saved in your appointment history for future reference.
From the Booking Link Creation and Setup page, click Automations at the top. Select Collect Information on the left menu. |

Under Manage Booking Information where it says How will you gather information when booking?, ensure that you have the option Ask for information with a web form selected. With this option selected, you should see Fields in Webforms below. This web form option has the following required information by default: First Name, Last Name, and Email Address. Note: The red asterisk inside the field indicates that the information is required and must be provided in order for the customer to complete the appointment booking process. |

Click the Customize fields or Order button to make changes to the default required fields. You'll see a list of information that you can enable and/or require from your customers. Tick the Enabled box next to the field you want to include in your web form. If you don't want to make it a required field, check the Optional box. |

If you’d like to add a custom field that isn’t already on the list, click the Add New Custom Field button. In the popup box, select whether the custom field should be required or optional. Create a name for your custom field in the Field Name box. Select the field type from the Type dropdown box. Click the Save button to create your custom field. |

When you’re done setting up the fields for your web form, click the Save and display summary button to apply the changes. You should now see your selected fields displayed under Fields in Webforms. When you’re happy with your settings, click the Save & Stay on this step button at the bottom-right side of the Booking Link Setup page. |
